The Health Resources and Services Administration (HRSA) announced on September 16, 2016 that Heartland Community Health Center has been awarded the Delivery System Health Information Investment award (DSHII) for $44,789. This one-time award will allow strategic investments in health information technology enhancements at the health center.

“Heartland remains on the front lines of progressive health initiatives and that requires enhancing the technological infrastructure to support our clinical work,” said Allie Nicholson, Heartland’s Operations Manager. “We’re grateful for grant support that allows us to better serve our patients.”

The purpose of the DSHII award is to:

• Accelerate health centers’ transition to value-based models of care
• Improve efforts to share and use information to support better decisions
• Increase engagement in delivery system transformation

“Health centers across the country are instrumental in providing high-quality, comprehensive primary health care to millions of people,” said US Health and Human Services Secretary Sylvia M. Burwell . “This investment will help unlock health care data and put it to work, improving health outcomes and building a better health care system for the American people.”

At Heartland, these IT enhancements will promote advanced accurate readings, efficiency in documentation, and improved provider outcomes. As a result, Heartland expects to see enhanced, accurate documentation, improved clinical satisfaction, improved patient satisfaction, increased portability, and the increased ability to encourage patient engagement.

For the second year in a row Heartland Community Health Center has received part of the Health Center Quality Improvement Award from the Health Resources and Services Administration (HRSA). The award is given to health centers all over the country to recognize quality care and to allow them to continue making improvements. Heartland received an award totaling $44,266.

More than $100 million has been awarded to health centers across the county to improve health center quality. The purpose of the awards is to:

  1. Recognize health centers that displayed high levels of quality performance in 2015 uniform data system reporting and/or significantly improved quality of care from 2014 to 2015.
  2. Provide support for those health centers to continue to strengthen quality improvement activities.
  3. To recognize and provide support for health centers with one or more sites with new and/or continued patient centered medical home recognition.

“It is always great to be rewarded for hard work, and that is exactly what this award is. We have continued to improve our quality through the hard work of our entire staff. We all contribute to the improved quality of our health center,” said Allie Nicholson, Heartland’s Operations Manager.

Heartland received $10,000 for its usage of electronic health records, $4,266 for notable improvement in clinical quality improvements, and $30,000 for achieving Patient Centered Medical Home recognition in 2015.

Heartland Community Health Center will begin offering primary care services in the fall of 2016 at the Lawrence-Douglas County Health Department’s clinic, located at 200 Maine St. These services are not currently offered at the Health Department. The goal is to make it more convenient for residents to access primary care services.

“By embedding a Heartland primary care team within the operation of the Health Department, our organizations will be better positioned and equipped to reach the families and individuals who currently do not have access to comprehensive primary care services,” said Heartland CEO Jon Stewart. “Our efforts here are to take primary care services to places where people already have established trusting relationships.”

About 10,000 people use the Health Department’s clinic services each year. A recent survey of 424 Health Department clients revealed that 46 percent did not have a regular health care provider. “We want to knock down some barriers to, hopefully, get people into a health home,” Health Department Director Dan Partridge said “Having a health home helps you do more in the preventive arena rather than waiting until you’re sick or you have some type of diagnosis or disease, and then it’s more costly to you and the health care system.”

The Health Resources and Services Administration (HRSA) announced on Thursday, June 16, 2016 that Heartland Community Health Center will be awarded Oral Health Service funds to increase access to integrated oral health care services and improve oral health outcomes for Health Center Program patients. Health and Human Services (HHS) Secretary Sylvia M. Burwell announced nearly $156 million in funding to support 420 health centers in 47 states. Among the awards is $350,000 to Heartland Community Health Center of Lawrence, one of only six awards made to Kansas Health Centers.

“We have worked hard at Heartland to acquire expertise in the delivery of interdisciplinary healthcare services,” said Jon Stewart, Heartland CEO. “We consider the team at Douglas County Dental Clinic to be experts in the delivery of dental services for the targeted population and we look forward to increased collaborative to provide high-quality oral health services for our community”

Heartland will be partnering with Douglas County Dental Clinic to bring a satellite clinic of dental services under Heartland’s roof. The funding enables health centers to expand integrated oral health care services and increase the number of patients served.

“Douglas County Dental Clinic is committed to partnering with Heartland to offer affordable, comprehensive dental services. Both organizations strongly believe these funds will serve to help alleviate an enormous need by offering affordable dental services in our community,” Julie Branstrom, Douglas County Dental Clinic Executive Director.

“Oral health is an important part of our overall physical health and well-being,” said Secretary Burwell. “The funding we are awarding will reduce barriers to quality dental care for hundreds of thousands of Americans by bringing new oral health providers to health centers across the country.”

The Health Resources and Services Administration (HRSA) announced on Wednesday, May 4 that Heartland Community Health Center will be awarded a grant from the Health Infrastructure Investment Program (HIIP). Health and Human Services secretary Sylvia M Burwell announced over $260 million in funding to 290 Community Health Centers nationwide. Among the awards is $1,000,000 to Heartland Community Health Center of Lawrence, one of only three awards made to Kansas Health Centers.

“We are thrilled and humbled by this announcement,” said Jon Stewart, Heartland CEO. “These funds will allow Heartland to purchase new exam room equipment, upgrade technology and renovate our facility to serve more patients. In turn, this will mean a better patient experience and improved access to high quality care.”

This investment builds on the nearly $150 million awarded to 160 health centers for construction and/or renovation in September 2015. This funding comes from the Affordable Care Act’s Community Health Center (CHC) Fund, which was extended with bipartisan support in the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015.

“Health centers are cornerstones of the communities they serve,” said Secretary
Burwell. “Today’s awards will empower health centers to build more capacity and provide needed health care to hundreds of thousands of additional individuals and their families.”

Heartland Community Health Center and the Lawrence-Douglas County Health Department are partnering to host another “Tools for Better Health” workshop beginning January 25. Meetings will be on Mondays from January 25 to February 29 at Just Food (1000 E. 11th St.) from 9:00am – 11:30am.

The class is seven weeks long and is designed for adults with chronic conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain, or depression. Information covered in the class will help participants find practical ways to deal with pain and fatigue, discover better nutrition and exercise choices, understand new treatment options, and learn better ways to talk with their doctor and family about their health.

Those who would like to attend, but need help with transportation, are encouraged to contact Heartland at 785.841.7297 for information on transportation vouchers.
