How Integrated Care Helped A Construction Worker Rebuild His Health
When James, a 42-year-old construction worker, first came to the Heartland Community Health Center in 2022, he had no idea his life was about to change. At the time, he was grappling with shortness of breath and lightheadedness while working on job sites—symptoms he chalked up to the physical demands of construction. But during a routine physical exam, Clare Kuhn, APRN-C, heard something unexpected: a heart murmur.
After hearing the murmur, Clare sent James for an echocardiogram and the results were alarming. James was found to have severe aortic insufficiency—a condition where the heart’s aortic valve fails to close properly, allowing blood to leak back into the heart. Clare referred him to LMH Cardiology, where they quickly uncovered the cause: infective endocarditis, a life-threatening infection linked to an untreated dental abscess. The infection had caused severe damage to James’ aortic valve.
With his condition worsening, James underwent a successful valve replacement surgery at St. Luke’s Hospital. He also had several dental extractions to address the underlying cause of the infection. Nearly a year later, his follow-up echocardiogram revealed incredible progress—his heart’s function had returned to normal, and his left ventricle, which had been enlarged, was back to a healthy size.
But James’ transformation didn’t stop with the surgery. Inspired by his second chance at life, he adopted a healthier lifestyle, making significant changes to his diet, quitting alcohol, and losing weight. Today, he radiates positivity and serves as a role model for others navigating health challenges.
“Seeing James today is like seeing a whole new person,” Clare shares. “He’s worked so hard to take care of himself, and it’s truly inspiring. His resilience reminds me why I do what I do.”
James’ journey reflects Heartland’s commitment to whole-person care. At Heartland, providers look beyond symptoms, addressing the root causes of illness and connecting patients to the care they need—whether through mental health support, dental services, or partnerships with community specialists like LMH Cardiology. This integrated approach ensures every patient receives the right care at the right time, with a focus on treating the whole person, not just the condition.
By working together across disciplines and partnering with community resources, Heartland’s team empowers patients like James to take charge of their health and live better, healthier lives. His story is a powerful reminder of how comprehensive, compassionate care can transform lives and inspire change.
