Primary Care & Family Medicine
With six family medicine providers, Heartland provides high-quality, patient-centered primary care to patients of all ages.
Services Include:
- Adult and child wellness visits & immunizations
- Diagnosis and treatment of acute illnesses and injuries
- Family Planning & Reproductive Health Care including hormone replacement therapy, screening for cervical cancer & sexually transmitted infections
- Gender Affirming Hormone Therapy
- Rapid HIV screening, pre and post-exposure HIV prophylaxis (PrEP, and PEP services)
- Medication assisted substance disorder treatment
- In-house lab and comprehensive send out labs
- Chronic illness management and speciality complex care
