Heartland’s community health workers are committed to helping you overcome any barriers to better health. Whether you need assistance finding stable housing, enrolling in educational programs, or accessing nutritious food, we are here for you.
We connect you with valuable resources and provide the support you need to improve your quality of life.
Our Community Health Worker Program offers individualized goal setting and service planning, linking you to financial aid and insurance resources, providing advocacy and support, coordinating with your case manager, and conducting outreach as a bridge to primary care.
At Heartland we believe food is medicine. That’s why we have an on-site pantry full of healthy foods and free nutrition classes.
In collaboration with Harvesters and our new Food is Medicine program, Heartland’s Just Food, Care Cupboard offers monthly classes the first Wednesday of each month.
These classes are focused on diabetes nutrition management through a myriad of topics. This is a great opportunity for the public to meet directly with a member of Harvester’s Nutrition team.
